I've slowly been making my custom neighborhood (like over a period of six months) I have used the help on advice on this site. Everything has been going fine.
I've just made some bar attendants. For some reason they won't show up behind the bar when I go to a downtown lot. I end up with maxis created ones. Yet if I hire one for a party at home the one I created will show up. I used the tree and create a bar attendant then made them over using SimPE. Do the downtown ones come under something else different to the homebar attendants? I can't seem to see anything else in the list of NPC's.

Yes, they're a different "type" of barmen.
I'm not sure how to spawn the community type barman, but if you're using the townie tree there's a separate menu for Nightlife NPCs, isn't there?  Community bars came with Nightlife.

Yes but they tree only has , DJ, Chef, Host and some other NPC I can't remember. Think it is the Gypsy.

Not yet checked the tree, but

Bartender = base game (that who can be hired for parties at home)
Barista = EP2 (NL) one - he attend the bar and the coffeemaker.

The coffe makers came with Uni.  Are the baristas the same as the community bartenders?

True! I forgot about that.
Just checked in game. Baristas came with Uni and not NL. You can find the barista NPC under "EP1 NPCs" in the tree npc maker.

I ended up just doing some sim surgery on the game created ones. Will changing the sex of a NPC through SimPE cause issues?

As long as they stay NPCs I don't think it'll cause major issues, but there's no telling what will happen if you move them in and try to play them.  I've heard great stories of trouble caused by messing with genders.
