I have three other servos who do not have this problem. It happens only when he's dressed in formal wear. It still happened when I removed my downloads folder. If it doesn't go when he changes back to everyday, try putting him in front of a mirror.
The male Servo in my servo couple has been in formal gear since he was present at a wedding. His wife was at the same wedding and she changed back into normal gear when she got out of bed, but he didn't. Maybe he just prefers to dress-up! Oh, and with a face like that, I'd get him changed back as soon as possible. I can tell you haven't got the alternative "not-quite-so-fugly" face templates in your game! And I do have face replacement templates, I have no idea where that ugly face came from. He's by far the ugliest face in my neigbourhood. I have not yet located the trigger for this, and it doesn't always happen consistently.
