Okay, so, this has only happened twice, and it's not so much melting as... I have no idea what to call it.
I had a sim go downtown the other day and everyone started smustling. I noticed one of the male smustlers head go see through in places (rather like Gappy's hair) and then his face was on the back uh... inside? of his skull. And then it happened to everyone else on the lot except my playable, so I had my sim run back home before she was afflicted.
Then in my legacy, my girl brought home her great great uncle from school, and proceeded to flirt with him. I canceled it out, and her head started going see through. So I reset her, and it's been fine ever since, which has been like... two days?
I have no pictures, unfortunately, hopefully someone knows what I mean.

This doesn't seem like a hack thing to me, but I'm an idiot, so I will tell you I have twojeff's samesexpregnancy. And that's it, this is a fairly new game.
(the weird part is that this is all in the family--the sim who went downtown was the founder, the incestuous teen was Gen 5. Hasn't happened to anyone else.)

What EPs and SPs do you have loaded? 
What are your system specs (esp. video card, RAM, processor, power supply wattage, etc.)?
Why are you playing a hack-free game?  You should at least have awesomeware critical fixes installed for the latest EP you have loaded.

Okay, so the freakiness happened again and I got a picture this time.
I think it's something to do with the portals. I also see it momentarily as sims enter and exit lots. But this so far has happened a. As a sim is getting off the bus and b. Right after they went downtown.

I may be wrong, but this looks like something that happened to me before. That had to do with the hair alpha transparency making my sim's head transparent. The FFS debugger can fix that. Click on your FFS debugger (I assume you have one) and go to the Fix... menu. Does the option "Invisible Sims" appear under the Fix... menu? If so click on it and that should fix it.
